Nihlathak, the elder who gave the Relic of the Ancients to Baal, waits at the end of his temple in the Halls of Vaught in Act 5. He teleports around, explodes the corpses near you and curses you with Horror. Killing him completes 
Betrayal of Harrogath, after which Anya personalizes one of your items.
In Hell, Nihlathak drops the 
Key of Destruction, one of the three keys that open the portals of 
The Pandemonium Event.
