Bloodthief is worth using for the 35% Chance of Open Wounds. Open Wounds scales with the level of the character who applies it rather than with weapon damage, so a proc at level 17 drains roughly 157 life over its eight seconds, several times what a single 11 to 28 hit from this spear does, and no amount of physical resistance reduces it. The 8-12% Life Stolen Per Hit is taken from damage actually dealt and therefore returns very little here.
