Brainhew is a unique Great Axe found in Diablo 2 Resurrected.

Brainhew carries 10-13% Mana Stolen Per Hit, and nothing with a larger roll becomes available until level 43, so for a long stretch it is the best mana steal a character can equip. A Great Axe hits hard enough for a percentage of the damage to be worth taking, which funds skill spam on a melee build well before mana on kill or a large mana pool becomes practical. Everything else on it is filler, so the axe is worth using only while mana is the thing limiting how often a skill fires.

How to upgrade Brainhew

You can upgrade Brainhew to a Gothic Axe with the following cube recipe:

1 Ral Rune + 1 Sol Rune + 1 Perfect Emerald + Brainhew

When upgrading Brainhew to a Gothic Axe the level requirement will increase to level 30.
