A Perfect Skull provides Attacker Takes Damage of 20 when inserted in shields or off-hands, Replenish Life +5, Regenerate Mana 19% in body armor and headgear, and 3% Mana stolen per hit, 4% Life stolen per hit in weapons.
A Perfect Skull requires level 18 to use. When you socket an item with a Perfect Skull, you increase the level requirement to level 18, unless it was already higher.

A Perfect Skull does not drop from any monsters or objects. It can drop from the Hellforge when destroying Mephisto's Soulstone, or from a gem shrine when holding a Flawless Skull.
